Typically, Mon-Fri; however, flexibility with schedule may be
required based on program needs.
Compensation: $25.00 - $27.00/hour

About Zack’s Crib: Zack’s Crib Crisis Centre (ZC) is a safe beds facility with the Mission to “promote, coordinate and provide services that alleviate and prevent homelessness.” Zack’s Crib Crisis Centre was opened to support the community in December 2023.
Position Summary: Zack’s Crib is seeking a compassionate and dedicated Intensive Case Manager to join our team. The ideal candidate will have a strong background in social work, housing services, and case management. This role involves working closely with clients to develop and implement individualized housing plans, providing ongoing support, and connecting them with necessary resources to achieve stable housing.
Key Responsibilities / Accountabilities:
- Conduct comprehensive assessments to identify clients' housing needs, strengths, and barriers. Conduct safety risk assessments and complete intake procedures as needed.
- Develop goals and implement individualized housing plans in collaboration with clients. Assist individuals with completing paperwork, attending appointments in the community, etc. as required to obtain and secure housing.
- Provide intensive case management services, including crisis intervention, advocacy, community outreach and support.
- Connect clients with community resources, such as housing programs, financial assistance, and mental health services.
- Provide information, referrals, and options for client’s housing, legal, medical, financial, cultural, social and wellness needs.
- Maintain accurate and up-to-date case files and documentation.
- Complete all documentation to professional standards and in a timely manner.
- Collaborate with other service providers, liaise with landlords, and community organizations to ensure coordinated care.
- Participate in committees, task forces, community collaborations and network with organizations to provide a voice for homelessness service needs.
- Monitor clients' progress and adjust housing plans as needed to achieve and maintain desired outcomes.
- Plan, organize, and facilitate group programming, as required to ensure programs run in a safe, inclusive and engaging manner.
- Represent Zack’s Crib in a professional manner at community and fundraising events.
- Participate in meal planning and preparation, cleaning / housekeeping duties, while maintaining an organized and sanitary environment as needed.
- Actively participate in team meetings, training sessions, supervision, ongoing professional development opportunities and remain current regarding best practices / supports for those experiencing homelessness.
- Maintain Health and Safety requirements. Maintain safety and security throughout the program.
- Maintain professional and ethical boundaries with clients and community partners. Adhere to and maintain confidentiality.
- Know and follow all policies and procedures.
- Other duties as assigned.

Qualifications and Requirements:
- Post-secondary education in Social Work, Psychology, Mental Health and Addictions, Social Justice & Community Development or a relevant field.
- Minimum 1 year of experience in case management, housing services, or a related field.
- Strong understanding of housing programs, community resources, and best practices.
- Excellent communication, organizational, and problem-solving skills.
- Ability to utilize effective conflict resolution and verbal de-escalation techniques.
- Ability to work independently and as part of a team.
- Compassionate, empathetic, and committed to supporting vulnerable populations and marginalized individuals.
- Proficiency in computers and Microsoft Office.
- Flexibility with schedule to meet program needs.
- Ability to work alongside animals periodically.
- Related shelter knowledge and/or lived is considered an asset.
- The ability to speak multiple languages will be considered an asset.
- CPI Certification, Mental Health First Aid, Trauma Informed Care Training and Safe Food Handling are considered an asset.
- Able to provide an acceptable Vulnerable Sector Police Check (VSC).
- Current Standard First Aid / CPR Certification or willingness to obtain at own cost.
Physical Demands:
The physical demands of the role may include but are not limited to lifting (up to 25kg), bending, twisting, reaching, sitting, or standing for extended periods, pushing, pulling, and moving constantly throughout the shift.
